reMELD-NA calculator
Since 25 March 2025, Eurotransplant uses the reMELD-Na (refitted MELD-Natrium) to calculate the labMELD score to assess the risk of patient’s mortality on the waiting list for liver transplantation. This score includes the 4 parameters serum creatinin, serum bilirubin, serum sodium and the INR. Of note, this score is NOT similar to the UNOS MELD-Na score, but was refitted for the Eurotransplant population. Therefore, general online calculators for MELD-Na score mostly refer to the UNOS/OPTN MELD-Na score and are not applicable for the Eurotransplant reMELD-Na. You can use the calculator below to calculate the reMELD-Na score for your patients.

For adult patients who have had dialysis twice within the last week, the creatinine value will be automatically set to 2.5 mg/dl / 221.1 umol/l.
For ReMELD-Na, the following lower and higher caps are used when calculating the score:
- Creatinine: 0.7-2.5 mg/dl / 61.9-221.1 umol/l
- INR: 1.0-2.6
- Bilirubin: 0.3-27 mg/dl / 5.13-461.7 umol/l
- Sodium: 120-138.6 mmol/l
Fill in 1 for INR if DOAC is used and anticoagulative effect is present.
Fill in 1 for INR if Vitamin K antagonist used but no value prior to start.
